Wisconsin Licensed Community Health Worker

Job Summary

A company is looking for a Community Health Worker to assess, plan, and implement individualized care strategies for members.

Key Responsibilities:

• Engage members primarily over the phone to discuss their health and create positive relationships
• Support members in managing their health and accessing community resources
• Conduct outreach to encourage participation in health-related programs and services

Required Qualifications:

• High School Diploma/GED (or higher)
• 1+ years of clinical or case management experience
• Intermediate experience using Microsoft Office (Word, Excel, Outlook)
• Must reside in Wisconsin and have a designated quiet workspace
• Availability to work a shift between 8:00 am - 5:00 pm CST
