Job Summary

A company is looking for a Patient Enrollment Specialist to advocate for patients considering in-home care services.

Key Responsibilities

• Initiate outreach to educate patients and caregivers on home-based care benefits
• Build rapport with patients, addressing objections and providing personalized support for enrollment
• Guide patients through the enrollment process, ensuring timely communication and continuity of care

Required Qualifications

• High school diploma or equivalent required; associate or bachelor's degree in a related field preferred
• 1-3 years of experience in healthcare outreach or patient-focused roles
• Prior experience in home health, hospice, or managed care preferred
• Comfort working with EMR systems; familiarity with Athena preferred
• Bilingual abilities (especially Spanish) are a plus
