Job Summary

A company is looking for an LPN Health Coach in Care Management.

Key Responsibilities

• Provide support for clinical outcome management and healthy lifestyle information to patients
• Complete transitions of care calls for moderate risk patients and document thoroughly
• Assist patients with health literacy, medication instructions, and follow up on care plans

Required Qualifications

• Certificate Program Post High School Vocational/Specialized Training Required
• 1 year of healthcare experience required
• Licensed Practical Nurse or Multi-State License upon hire required
• Basic Life Support certification upon hire required
• Completion of all required education programs and competency testing upon hire required
