Registered Nurse Transplant Case Manager

Job Summary

A company is looking for a Transplant Case Manager, RN.

Key Responsibilities

• Manage transplant cases and coordinate care using approved clinical guidelines
• Conduct clinical assessments and provide educational resources to members regarding their health conditions
• Monitor and evaluate the effectiveness of care plans and document all case management activities

Required Qualifications

• Graduate of an accredited school of nursing
• Minimum of three years of transplant experience required
• At least one year of experience in Utilization Management or Case Management
• Current, active, and unrestricted compact Registered Nurse license
• Intermediate knowledge of Microsoft Office applications
