Job Summary

A company is looking for an Assessment, Care Manager.

Key Responsibilities

• Conduct assessments to understand caregiver needs
• Provide technical and emotional support to caregivers
• Offer guidance and connect caregivers with community resources

Required Qualifications

• Active licensure as a Licensed Social Worker (LSW, LISW, LCSW), Licensed Practical Nurse (LPN), or Registered Nurse (RN)
• Strong analytical and troubleshooting abilities
• Experience in customer support and technical support
• Ability to work independently and remotely
• Experience in healthcare or caregiving is a plus
