Multi-State Licensed Diabetes Nurse Practitioner

Job Summary

A company is looking for a Diabetes Lead Nurse Practitioner to oversee clinical and operational aspects of a diabetes care program while providing patient care.

Key Responsibilities

• Manage and oversee the performance of the diabetes NP team, including quality reviews and coaching
• Provide clinical care to patients with diabetes as part of an interdisciplinary team
• Identify areas for improvement and implement strategies to enhance patient care

Required Qualifications and Education

• Master's or Doctoral degree in Nursing with a specialization in primary care or cardiovascular care
• Active RN and APRN license, along with APRN certification, required at the time of hire
• Five or more years of clinical practice experience as a nurse practitioner, with extensive diabetes/endocrinology experience
• Experience with telehealth technology and electronic health records (EHR)
• Multi-state licensure required, with a willingness to obtain and maintain additional licenses as needed
