Job Summary

A company is looking for a Complex Oncology Nurse Navigator.

Key Responsibilities

• Conduct clinical assessments, monitor health changes, and coordinate care for members diagnosed with cancer
• Educate members and caregivers about diagnosis and treatment while providing telephonic assessments and support
• Collaborate with healthcare teams and assist with care coordination, discharge plans, and addressing social determinants of health

Required Qualifications

• Bachelor of Science in Nursing (BSN) and an unrestricted Registered Nurse (RN) license
• Minimum of 5 years of nursing experience, including 3 years in high-acuity adult oncology
• OCN certification required at time of hire or commitment to obtain within 2 years
• Ability to manage multiple tasks and work under pressure while maintaining organization
• Comfort with technology and learning new tools relevant to patient care
