Job Summary

A company is looking for a Care Manager Transitions of Care.

Key Responsibilities

• Evaluate member needs through post-discharge assessments and medication reconciliation
• Develop and implement care/service plans in collaboration with healthcare teams
• Facilitate member transitions into active care management and provide educational resources

Required Qualifications

• Master's degree in Behavioral Health or Social Work, or a degree from an accredited school of nursing
• 2 - 4 years of related experience in care management
• Active RN license is required
• Experience with compliance in care management activities
• Ability to collaborate with interdisciplinary teams and external resources
