Tennessee Licensed RN Care Manager

Job Summary

A company is looking for a Transitions of Care (TOC) RN Care Manager.

Key Responsibilities

• Conduct Transition of Care assessments to identify patient needs and gaps in care
• Create and implement care plans in collaboration with patients and the care team
• Coordinate care and manage patient transitions to minimize avoidable readmissions

Required Qualifications

• Bachelor's degree in nursing preferred
• 3-5 years of nursing experience in case management or care management
• Current, unrestricted compact Registered Nurse license
• Valid BLS certification from a licensed AHA or American Red Cross training facility
