Job Summary
A company is looking for an Access Center Navigator (Per Diem).
Key Responsibilities
- Serve as a point of contact for patients, caregivers, and referring physicians to facilitate appointments and referrals
- Navigate patients through the continuum of care and coordinate appointments, consultations, and classes
- Maintain accurate patient records and follow up with patients while collaborating with care teams
Required Qualifications
- 2-3 years of customer service or call center experience
- High school degree or equivalent; associate's degree preferred
- Healthcare experience, including knowledge of medical terminology and managed care, strongly preferred
- Ability to work quickly and make accurate decisions
- Familiarity with general office equipment, including multi-line telephone systems
