Job Summary

A company is looking for a Care Advisor, Care Concierge.

Key Responsibilities

• Assist members with assessments and recommend service plans based on individual needs
• Coordinate and schedule services while following up with members
• Support members in accessing community resources and benefits

Required Qualifications

• High School Diploma preferred
• Experience in Public/Community Health or a related field preferred
• Experience in Social Services, Care Coordination, or Healthcare Navigation is a plus
• Must be 18 years of age or older
• Computer proficiency and knowledge of clerical functions required
