Key Responsibilities
- Engage and educate caregivers through phone and digital technology to enhance their wellbeing
- Manage a caseload and assess caregiver and care recipient needs, assisting in care plan goal achievement
- Document coaching activities and collaborate with stakeholders to identify problems and solutions
Required Qualifications, Training, and Education
- 4+ years of experience in care planning or case management
- Experience working with the elderly or adults with disabilities
- Remote case management experience is highly desired
- Comprehensive knowledge of industry concepts and principles
- Ability to navigate and teach others to use mobile applications
