A company is looking for a Care Coach II to support family caregivers and their care recipients.

Key Responsibilities

• Engage and educate caregivers through phone and digital technology to enhance their wellbeing
• Manage a caseload and assess caregiver and care recipient needs, assisting in care plan goal achievement
• Document coaching activities and collaborate with stakeholders to identify problems and solutions

Required Qualifications, Training, and Education

• 4+ years of experience in care planning or case management
• Experience working with the elderly or adults with disabilities
• Remote case management experience is highly desired
• Comprehensive knowledge of industry concepts and principles
• Ability to navigate and teach others to use mobile applications
