Job Summary
A company is looking for a Care Coordinator to support members through telephonic interactions and collaboration with clinicians.
Key Responsibilities:
- Handle inbound and outbound calls to support members
- Complete intake screening questionnaires and audio/video initial program assessments with members
- Conduct cold calls for member engagement and assist with scheduling appointments
Required Qualifications:
- 2+ years of healthcare/clinical experience
- Call agent experience is preferred
- Basic understanding of EHR/EMR applications
- Proficiency in Microsoft Office
- Must live in the United States
