Care Delivery Manager

Job Summary

A company is looking for a Manager of Care Delivery to lead a team of Care Advocates and Care Coaches.

Key Responsibilities

• Conduct regular one-on-ones, performance reviews, and development planning for team members
• Ensure a culture of empathetic care while achieving key performance indicators related to clinical outcomes and member satisfaction
• Collaborate with cross-functional teams and lead member care-related projects from conception to execution

Required Qualifications

• 5-7 years of experience in allied health, human services, or healthcare organizations, with at least 3 years in a management role
• Experience in family planning and maternal-child health areas
• Strong sense of empathy towards team members, members, and providers
• Ability to make data-driven decisions and motivate a team to achieve KPIs
• Experience in managing change and creating order in a dynamic environment
