A company is looking for a Care Management Support Coordinator II.
Key Responsibilities
- Conduct outreach to members to assist with care plan steps and connect them to resources
- Serve as the primary contact for member and provider inquiries, explaining care plan procedures
- Document and maintain non-clinical member records in compliance with state and regulatory requirements
Required Qualifications
- High School diploma or GED
- 1 - 2 years of related experience in a call center, healthcare, insurance, or sales environment
- Knowledge of health plan activities and community resources
- Experience with member onboarding and administrative support
- Ability to work remotely in MST or CST time zones
