Job Summary

A company is looking for a Care Management Support Coordinator II.

Key Responsibilities
  • Conduct outreach to members to assist with care plan steps and connect them to resources
  • Serve as the primary contact for member and provider inquiries, explaining care plan procedures
  • Document and maintain non-clinical member records in compliance with state and regulatory requirements
Required Qualifications
  • High School diploma or GED
  • 1 - 2 years of related experience in a call center, healthcare, insurance, or sales environment
  • Knowledge of health plan activities and community resources
  • Experience with member onboarding and administrative support
  • Ability to work remotely in MST or CST time zones
