Job Summary
A company is looking for an Analyst, Case Management - Work From Home.
Key Responsibilities
- Assess members' needs and document interactions in electronic records to ensure compliance
- Connect members to community and national resources, and provide education on available services
- Support members during enrollment calls and assist in problem-solving when services are unavailable
Required Qualifications
- At least 2 years of experience in healthcare, nonprofit, or community-based services
- Experience with high call volume and auto-dialer systems
- Preferred experience in information & referral, health coaching, care management, or social work
- Bachelor's degree or equivalent work experience
- Bilingual (Spanish) preferred
