Job Summary

A company is looking for a Part Time Triage Support RN.

Key Responsibilities

• Provide non-face-to-face telephone triage and care planning for Medicare patients with chronic conditions
• Document encounters in patient charts and collaborate with healthcare teams to optimize patient care
• Educate patients on health goals and manage the Nurse Urgent Queue to ensure timely follow-up and coordination of care

Required Qualifications

• Current COMPACT license as a Registered Nurse (RN)
• Ability to work Monday - Friday from 5:00 pm to 9:00 pm EST
• Willingness to work an on-call rotating schedule and obtain multi-state licenses
• Familiarity with chronic care conditions and related medications
• Excellence in following clinical triage protocols and documenting in patient charts
