Disease Management Nurse - Compact License

Job Summary

A company is looking for a Disease Management Nurse - Remote.

Key Responsibilities

• Provide telephonic and omni-channel support to participants in the Disease Management program
• Educate participants about their chronic conditions and assist in developing personalized care plans
• Identify at-risk participants and facilitate referrals to appropriate healthcare resources

Required Qualifications

• Current Registered Nurse multi-state compact license; BSN preferred
• Minimum of 3 years recent clinical experience
• Basic knowledge of physiology and chronic disease management
• Ability to utilize basic computer skills and learn new software
• Must work from a home office that complies with HIPAA requirements
