Job Summary

A company is looking for a Virtual Primary Care Nurse Practitioner.

Key Responsibilities

• Provide virtual telehealth services, conducting video visits to assess patient needs and provide clinical guidance
• Serve as a consulting clinician to develop care plans and drive successful outcomes
• Manage a panel of primary care patients, delivering comprehensive care including acute and chronic disease management

Required Qualifications

• At least five years of experience in primary care within an ambulatory or outpatient setting
• Licensed Nurse Practitioner (NP) in good standing with the medical board of the state
• Current and active board certification from a national certifying body
• Experience managing a patient panel independently
• Must maintain Medicare active status and comply with security and HIPAA regulations
