Job Summary

A company is looking for a PT Clinical Navigator.

Key Responsibilities

• Conduct clinical intake assessments and provide patient education on treatment options
• Monitor patient progress and address barriers to care through personalized strategies
• Document patient interactions accurately and collaborate with care teams for seamless experiences

Required Qualifications

• Doctor of Physical Therapy (DPT) or Master of Physical Therapy (MPT) required
• Current, unrestricted PT license in Georgia (Compact License preferred)
• Minimum 2 years of relevant experience in physical therapy or MSK-related roles
• Willingness to obtain additional licensure in new states as needed
• High comfort level with technology and experience using data to optimize clinical workflows
