Job Summary

A company is looking for a Lead Care Manager to coordinate care for clients with complex needs.

Key Responsibilities
  • Assess member needs and develop client care plans
  • Connect clients to necessary social services and advocate on their behalf
  • Maintain documentation and evaluate client progress within established timeframes
Required Qualifications
  • 2+ years of experience as a care manager, care navigator, or community health worker
  • Experience working in a remote setting
  • Knowledge of government and community resources related to social determinants of health
  • General computer skills and familiarity with Google Workspace and MS Office
  • Bilingual (English/Spanish) is a plus
