Key Responsibilities
- Assess member needs and develop client care plans
- Connect clients to necessary social services and advocate on their behalf
- Maintain documentation and evaluate client progress within established timeframes
Required Qualifications
- 2+ years of experience as a care manager, care navigator, or community health worker
- Experience working in a remote setting
- Knowledge of government and community resources related to social determinants of health
- General computer skills and familiarity with Google Workspace and MS Office
- Bilingual (English/Spanish) is a plus
