LPN Care Manager (MI, VA, MD)

Job Summary

A company is looking for a Care Manager, LPN (MI, VA, MD).

Key Responsibilities

• Perform comprehensive chart reviews and reconcile medications
• Develop and implement personalized care plans for patients
• Monitor and assess patient progress, adjusting care plans as necessary

Required Qualifications

• Must be a Licensed Practical Nurse (LPN) with a compact license in good standing
• Proficient in using computers and various software applications
• Self-motivated and able to work independently in a remote setting
• Familiarity with chronic care management and geriatric care is preferred
• Previous experience in skilled nursing, assisted living, or long-term care settings is preferred
