Medicare Care Coordinator Associate

Job Summary

A company is looking for a Care Coordinator Associate Medicare.

Key Responsibilities

• Provides care coordination services to low-risk/low-acuity members
• Conducts surveys and assessments with Enrollees and/or their caregivers
• Creates and updates Individualized Care Plans and conducts medication reviews

Required Qualifications

• Bachelor's Degree in a health or social services-related field preferred
• 1 to 3 years of social services and/or care management experience preferred
• 3 to 5 years of Case Management experience preferred
• Strongly prefer Medical Assistant, Home Health Aide, or Nursing Assistant experience
• Experience in a remote/telephonic environment preferred
