Job Summary
A company is looking for a Triage & Transitions Ambulatory Registered Nurse.
Key Responsibilities
- Manage patient calls requiring triage and coordinate care during transitions post-inpatient stay
- Provide guidance, education, and resources for managing at-risk patient populations
- Perform clinical triage, including assessments and patient education using established protocols
Required Qualifications, Training, and Education
- ASN/ADN from an accredited program with the ability to obtain a BSN within four years or 15 years of RN experience in acute care
- Current Montana RN license or compact license with the ability to obtain an RN license in the state of practice
- Current BLS certification endorsed by the American Heart Association
- Preferred: Bachelor's Degree in Nursing (BSN) from an accredited institution
- Preferred: One year of experience in a clinic or inpatient acute hospital setting
