Job Summary

A company is looking for a Nurse Practitioner to support complex patients in navigating the healthcare system remotely.

Key Responsibilities

• Complete comprehensive health assessments for patients
• Manage complex patients to reduce hospitalizations and improve health outcomes
• Work autonomously and collaboratively with the healthcare team to ensure quality care

Required Qualifications

• Minimum of two (2) years' experience as a Nurse Practitioner (NP)
• Current unrestricted NP license to practice in the state
• Current CPR or BLS certification
• Intermediate computer skills and proficiency in MS Word, Excel, and Outlook
• Willingness to obtain up to 6 additional state licenses for remote patient management
