Job Summary

A company is looking for a NP Licensed in NY for a Telephonic On Call remote position.

Key Responsibilities:

• Provide telephonic care and direction to members and facility staff during various hours
• Manage medical and behavioral conditions in collaboration with physicians or specialty providers
• Document encounters appropriately and monitor the needs of members and families for care coordination

Required Qualifications:

• Active NY NP license or ability to obtain by start date
• Active national certification in Family, Geriatrics, Adult, or Adult-Gerontology
• Current active DEA licensure or ability to obtain post-hire
• Availability to work assigned weekends, weekdays, and pre-assigned holidays
