North Carolina Discharge Planning Coordinator

Job Summary

A company is looking for a Discharge Planning Coordinator to support members transitioning from hospital or crisis services to community resources.

Key Responsibilities

• Serve as a point of contact for discharge planning and coordination of care needs following hospital admissions
• Provide short-term care coordination and facilitate safe and timely discharges
• Link members and stakeholders to various services, providers, or community resources

Required Qualifications

• High School Diploma/GED with six years of relevant experience; or an Associate's degree with four years of experience; or a Bachelor's degree with two years of experience; or equivalent combination of education and experience
• Valid driver's license
• Residency in North Carolina
• Ability to travel within the catchment area as required
