New York State Licensed Care Manager

Job Summary

A company is looking for a Care Manager, Fluent in English/Fujianese - 100% Remote.

Key Responsibilities:

• Advocates for and educates members on health services and self-management techniques
• Conducts assessments to identify barriers and develops care plans aligned with treatment goals
• Coordinates with healthcare teams and community resources to ensure effective care delivery

Required Qualifications:

• NYS RN or LCSW/LMSW (any state) for Medical Care Management
• 3 years of experience in a mental/behavioral health or addictions setting for Behavioral Health Care Management
• 1 year of pediatric clinical experience for PEDS positions
• Fluency in Fujianese is preferred
• Experience managing member information in a shared network environment
