Key Responsibilities
- Coordinates intake of cases and facilitates the Care Coordination process
- Researches and maintains resources, guidelines, and policies for the team
- Assists in the development and training of new department members
Required Qualifications, Training, and Education
- High School Graduate
- Typically requires 5 years of experience in population health management or related fields
- Proficiency in ICD-10, CPT, and HCPCS coding preferred
- Knowledge of medical terminology
- Advanced skills in Microsoft Office applications
