Registered Nurse Case Manager

Job Summary

A company is looking for a Nurse Case Manager - Maternity (CST/MST/PT preferred).

Key Responsibilities

• Assess members' needs and develop individualized care plans
• Monitor care plans and coordinate with providers for service delivery
• Provide education and referrals to community resources for members

Required Qualifications

• Registered Nurse (RN) with a current, unrestricted license
• Preferred college degree
• Bilingual in Spanish is strongly preferred
• Three or more years of experience in a Case Management role
• CCM certification required within specified timeframe
