A company is looking for a Care Management Registered Nurse, RN Compact License Required.

Key Responsibilities

• Continuously monitor patient vitals, symptoms, and lab results to identify care gaps
• Create and manage personalized care plans aligned with treatment goals
• Conduct virtual follow-up appointments and document clinical interactions thoroughly

Required Qualifications, Training, and Education

• Multi-state RN Compact State Licensure
• Associate Degree in Nursing Science
• 5+ years of clinical experience as an RN in an ICU/ER setting
• Prior experience in chronic care management is a plus
• Technical fluency with multiple platforms and systems
