Job Summary

A company is looking for a Social Care Navigator (Mandarin/Cantonese Speaking).

Key Responsibilities

• Utilize motivational interviewing to empower members in achieving health and personal goals
• Conduct outreach to understand member needs and provide tailored support
• Identify and refer members to local resources and collaborate with internal teams for continuity of care

Required Qualifications

• 2+ years of experience in Care Navigation, Social Work, Health Coaching, or related fields
• Fluency in Mandarin or Cantonese is required
• Experience with Motivational Interviewing or similar techniques
• Familiarity with systems like Medicaid, SNAP, and public housing
• Comfortable working remotely and using digital tools for organization and documentation
