Job Summary

A company is looking for a PCMH LPN Care Coordinator.

Key Responsibilities

• Identify patients needing care coordination and arrange follow-up services
• Communicate with patients and care teams to ensure continuity of care
• Identify patient needs and coordinate with community resources for support

Required Qualifications, Training, and Education

• Graduation from an accredited Practical Nurse program
• 5 years of relevant medical office experience
• Current South Carolina LPN license or compact state license
• Current Basic Life Support (BLS) certification
• Proficiency in MS Word, Excel, and PowerPoint; working knowledge of electronic medical records
