Job Summary

A company is looking for a Spanish Speaking LPN / LVN.

Key Responsibilities

• Facilitate patient enrollment into RTM and CCM programs
• Make daily calls and text messages to document patient health status
• Assist in the collection and documentation of patient health information

Required Qualifications, Training, and Education

• Certification as an LPN
• 3+ years of experience working in office or remote
• Proficiency with technology such as Zoom and Slack
• Familiarity with HIPAA regulations
• Ability to work independently in a remote setting
