Value Based Care Navigator

Job Summary

A company is looking for a Value Based Care Patient Navigator.

Key Responsibilities

• Serve as a point of contact for patients with unmet preventive or chronic care needs
• Educate patients on the importance of various health services and schedule appointments
• Document patient interactions and monitor care gaps while supporting program initiatives

Required Qualifications

• A minimum of a High School diploma/GED is required
• Experience working with patients in a healthcare or community-based setting is preferred
• Previous customer service experience is preferred
• Bilingual or multi-lingual skills appropriate to the patient population served are a plus
• Knowledge of software applications such as Microsoft Office and electronic medical record systems
