Weekend Registered Nurse Coordinator

Job Summary

A company is looking for a Part-Time Weekend Registered Nurse Coordinator (Alerts).

Key Responsibilities

• Monitor patient vitals, symptoms, and labs to identify patients needing clinical interventions
• Follow up with patients with abnormal readings and lead virtual follow-ups to support treatment plans
• Respond to inbound patient clinical questions and support clinical documentation for care summaries

Required Qualifications

• 4+ years of experience treating patients with CHF, hypertension, and diabetes
• Compact multi-state licensure (RN compact)
• Experience in a CHF bridge clinic environment
• Experience with remote patient monitoring technology
• Independent thinker with a passion for improving healthcare through digital innovation
