Required Qualifications

• 3-5 years of experience in social work, nursing, or a healthcare-related field
• Knowledge of referral coordination to community and private/public resources
• Bilingual in English and Spanish
• Experience in relationship management
• Highly organized and tech-savvy with effective time management skills
