Job Summary

A company is looking for a Health Outreach Specialist to connect with Medicare members and support their healthcare journeys.

Key Responsibilities

• Provides telephonic outreach to members post-discharge to offer support and referrals
• Engages members in discussions about case management program benefits and enrollment
• Coordinates calls and activities related to members' treatment plans and maintains a daily calling schedule

Required Qualifications

• High School degree or equivalent
• 1+ years of medical assistant or customer service experience in the medical field
• Experience in working with chronic disease states
• Ability to motivate people to participate in special programs
• Proficient in Microsoft Word and Excel
