Key Responsibilities
- Provides telephonic outreach to members post-discharge to offer support and referrals
- Engages members in discussions about case management program benefits and enrollment
- Coordinates calls and activities related to members' treatment plans and maintains a daily calling schedule
Required Qualifications
- High School degree or equivalent
- 1+ years of medical assistant or customer service experience in the medical field
- Experience in working with chronic disease states
- Ability to motivate people to participate in special programs
- Proficient in Microsoft Word and Excel
