Job Summary
A company is looking for a Telephonic Nurse Case Manager to coordinate patient care for high-risk individuals.
Key Responsibilities
- Establish collaborative relationships and act as an intermediary among clients, patients, employers, providers, and attorneys
- Assess, coordinate, implement, monitor, and evaluate care plans for injured/disabled individuals
- Document ongoing progress of case management activities and maintain clinical documentation
Required Qualifications
- RN License, preferably in a compact state and with an IL license
- At least 3 years of Telephonic Case Management experience preferred
- Experience with Workers' Compensation or Disability Management
- Associate degree required; BSN or CCM certification is a plus
- Strong computer skills, particularly in Microsoft Suite
