Job Summary

A company is looking for a Pharmacy Population Health Technician.

Key Responsibilities

• Complete patient outreach through telephone calls and electronic communication
• Assess and identify reasons for medication non-adherence and educate patients on improvement strategies
• Coordinate with stakeholders to resolve issues related to medication non-adherence and document interactions in the patient's Epic medical record

Required Qualifications

• Must have an Indiana Board of Pharmacy Technician license
• Passionate about engaging with patients
• Self-motivated with excellent time management skills
• Highly flexible in adapting to changing work environments
