LPN Transition Specialist

Job Summary

A company is looking for a Transition Specialist to manage and coordinate the transition of members in the community.

Key Responsibilities

• Support care coordination teams to develop effective transition plans for members
• Assist with member transitions based on enrollment or care service changes
• Track and maintain transition metrics to identify trends and process improvements

Required Qualifications

• Bachelor's degree and 2-4 years of related experience or equivalent experience
• LPN/LVN certification preferred
• Experience in care coordination or health services
• Knowledge of community health resources and transition services
• Ability to develop educational materials and training programs
