Key Responsibilities
- Support care coordination teams to develop effective transition plans for members
- Assist with member transitions based on enrollment or care service changes
- Track and maintain transition metrics to identify trends and process improvements
Required Qualifications
- Bachelor's degree and 2-4 years of related experience or equivalent experience
- LPN/LVN certification preferred
- Experience in care coordination or health services
- Knowledge of community health resources and transition services
- Ability to develop educational materials and training programs
