Job Summary

A company is looking for a Medication Navigator.

Key Responsibilities

• Engage with patients, providers, and pharmacies to ensure timely medication fills and refills
• Educate patients on their medication regimens and assist in overcoming adherence barriers
• Support quality performance measures and participate in strategy meetings with leadership and payors

Required Qualifications

• High School Diploma / GED (or higher)
• 1+ years of experience working with patients, preferably in medication management
• Basic proficiency with Microsoft Office products, including Word and Outlook
• Access to a designated quiet workspace with the ability to secure Protected Health Information (PHI)
• Must reside in an area with access to a high-speed internet connection
