Key Responsibilities

• Communicate with clients and families to identify barriers to aging in place and reduce expenses
• Utilize motivational interviewing techniques to engage individuals with varying health needs
• Document communication and intervention activities accurately and consistently

Required Qualifications

• Strong background in geriatric case management or coordination experience required
• Experience in social services, care coordination, healthcare navigation, or public health preferred
• 1 year of relevant experience in a customer service or similar role preferred
• Bachelor's degree in Psychology or Social Work preferred
• Must be 18 years of age or older
