Job Summary

A company is looking for a Telehealth Physician to provide remote care for Medicare patients.

Key Responsibilities

• Provide routine telehealth visits with Medicare patients, focusing on E/M and Chronic Care Management
• Create and manage personalized care plans for ongoing conditions
• Collaborate with an integrated care team and ensure thorough documentation in the EHR

Required Qualifications

• MD or DO, Board Certified/Eligible in Family Medicine, Internal Medicine, or Geriatrics
• Active medical license; multi-state licensure is a plus
• Credentialed with major insurance plans such as UHC, Aetna, and Humana
• Strong interest in preventive care and chronic condition management
• Tech-savvy and comfortable practicing via telehealth
