Job Summary

A company is looking for an LPN Hospice Intake Coordinator.

Key Responsibilities

• Receives and reviews patient referrals to hospice services, ensuring compliance with coverage criteria
• Coordinates the evaluation/admission process and communicates pertinent patient information to relevant parties
• Meets productivity expectations with quality documentation of complete referrals and performs other duties as assigned

Required Qualifications, Training, and Education

• Current, unencumbered Multistate license to practice as a licensed practical/vocational nurse
• Familiarity with MS Office, particularly Excel
• Reliable internet service and ability to work from home
• Availability for 12-hour shifts primarily every other weekend and scheduled weekdays
• Preferred hospice experience and two years of administrative experience in a healthcare environment
