Job Summary

A company is looking for a LTSS Service Care Manager in McAllen or Mission, TX.

Key Responsibilities

• Develop, assess, and coordinate holistic care management activities for long-term care members
• Monitor and evaluate care plans, member status, and outcomes while facilitating access to services
• Provide resource support and education to members and their families regarding healthcare options and benefits

Required Qualifications

• Bachelor's degree with 30 semester hours or equivalent in a human services field
• At least two years of experience in a relevant field
• Licensure or certification as required by state regulations
• Experience with long-term care management and service planning
• Knowledge of compliance with state, federal, and third-party payer regulations
