Associate Manager, Care Navigation

Job Summary

A company is looking for an Associate Manager, Care Navigation (Remote - Dementia Care).

Key Responsibilities

• Oversee daily activities of Care Navigators, ensuring quality standards and support
• Monitor operational workflows and track patient outreach and follow-up tasks
• Collaborate with clinical teams to support patient care plans and implement process improvements

Required Qualifications

• Bachelor's degree in Healthcare, Health Administration, Business, or a related field, or equivalent experience
• 3-5 years of experience in healthcare operations, preferably in geriatrics or dementia care
• At least 1 year in a lead or supervisory role
• Strong proficiency with EHR systems, G-Suite, and telehealth tools
• Knowledge of HIPAA and clinical compliance standards
