Job Summary

A company is looking for a Care Manager - BH.

Key Responsibilities

• Monitor inpatient and outpatient care services related to mental health and substance abuse treatment
• Provide telephone triage, crisis intervention, and develop discharge plans in collaboration with providers
• Participate in quality improvement activities and advocate for patient treatment needs

Required Qualifications, Training, and Education

• Current licensure in California as CEAP, LMSW, LCSW, LPC, RN, or LPN
• Minimum of 2 years of experience in healthcare, behavioral health, or substance abuse settings
• Knowledge of utilization management procedures and mental health community resources
• Experience in inpatient and outpatient settings
• Education requirements depend on certification, including Associate's in Nursing or Bachelor's/Master's in Social Work
