Required Qualifications

• 3 years of experience in healthcare coordination or a related field
• Experience with a team-based approach to care
• Strong knowledge of community resources
• Ability to prioritize tasks effectively in a remote environment
• Intermediate skills with MS Office Suite, including Outlook and Teams
