Key Responsibilities
- Provides outreach to members for care plan support and resource connections
- Serves as front-line support for member and provider inquiries regarding care plan procedures
- Documents and maintains non-clinical member records in compliance with regulatory standards
Required Qualifications
- High School diploma or GED required
- 1 - 2 years of related experience preferred
- Knowledge of health plan activities and resources is beneficial
- Understanding of Social Determinants of Health (SDOH) needs
- Ability to comply with policies and standards
