Job Summary

A company is looking for a Care Management Support Coordinator II.

Key Responsibilities
  • Provides outreach to members for care plan support and resource connections
  • Serves as front-line support for member and provider inquiries regarding care plan procedures
  • Documents and maintains non-clinical member records in compliance with regulatory standards
Required Qualifications
  • High School diploma or GED required
  • 1 - 2 years of related experience preferred
  • Knowledge of health plan activities and resources is beneficial
  • Understanding of Social Determinants of Health (SDOH) needs
  • Ability to comply with policies and standards
